Concussion Protocol Follow Up

In order to ensure the safety of all students, in the case of any head injury (ie. collision or fall), concussion protocol will be administered to access the student and check for signs or symptoms of a possible concussion.  If the student is not showing any signs or symptoms at the time of injury a phone call will be made notifying the parent of the incident and a copy of Black Gold Form 319-1 No Observed Signs or Symptoms – Documentation of Concussion Monitoring/Medical Examination will be sent home along with information on what to watch out for as far as a possible concussion is concerned.  The parent MUST check off the appropriate box and sign and date the form and return it to the school when their child returns.  The student is not to return to school for a 24 hour period and will be marked as a medical absence.  This allows for the parents to monitor for any signs of a possible concussion and seek medical attention if necessary.

If any signs of a concussion are present at the time of injury, a phone call will be made notifying the parent of the incident and a copy of Black Gold Form 319-2 Observed Signs or Symptoms – Documentation of Concussion Monitoring/Medical Examination will be sent home to be completed by the parent and a doctor.  This form MUST be completed before the child is allowed to return to school.
