Medical Procedure Request Waiver

A Medical Procedure Request Waiver MUST be filled out for students requiring short-term and long-term medication. Please return the completed form to the office and they will determine where the medication should be kept and who should administer it.

Any medication sent to the school without a completed form WILL NOT be administered to the child.
